Participation Requirments:

Since this is a group insurance, all qualified barangay tanods ang security guards are required to participate and enroll at the same time.
Minimum of 25 members.

Basic CLAIMS Requirements:

Endorsement Letter from the cooperative
Death Certificate (Original or Certified true copy from the original duly signed by the Local Civil Registrar)
Proof of Beneficiaries (Marriage Certificate or Birth Certificate of Children, etc.)
Police Report or Affidavit of Eye Witness original copy (For accident claim)
Claimant Statement
Identification
Attending Physical Statement (If died in the hospital) or Barangay Certificate or Investigative Report starting he place of death (If died at home)
Proof of Remittance and Payment from the cooperative.
